What is a fax cover sheet?
A fax cover sheet identifies the sender, recipient and purpose of a fax and can provide handling instructions. A useful cover sheet commonly contains sender and recipient names, fax numbers, date, subject, total page count, an optional message and an appropriate confidentiality notice.

Which template should you use?
The three layouts differ in how much they say before the recipient reaches the first page, not in how they look. Pick on the basis of what the fax is.
Standard
Sender and recipient details, date, subject and page count. The default, and the right one for most business correspondence — an order, an invoice query, a signed form going back to someone expecting it.
Simple
The addressing fields and little else. Useful when the recipient already knows what is coming and the cover sheet exists only so the machine at the other end puts the pages on the right desk.
Confidential
Adds a prominent confidentiality notice above the message area. Reach for it when the pages behind it contain personal, financial, medical or legal information and the fax is going to a shared machine.
Worth being clear about what that notice does. It communicates an expectation to whoever picks the pages up. It does not encrypt anything, does not control what happens to the pages after they print, and does not by itself satisfy any regulatory obligation you may be under. If a rule governs the information you are sending, confirm what that rule actually requires — the notice is a courtesy, not a control.
Making one without Word
The usual route to a cover sheet is a word processor template: open Word, search the template gallery, fill in a document, then print or export it. That works, and it needs the application installed, the template downloaded, and a file saved somewhere you will have to find again.
